Intake Form & Booking
To ensure I can provide the safest and most personalized care, all clients are required to complete the Client Intake form before scheduling their first appointment.
Once your intake form has been received and reviewed, and I have determined that the services requested are appropriate for your needs and within my scope of practice, I will send you the link to book your session online and/or reach out directly with upcoming dates and times.
If I believe another practitioner would better meet your needs, I am happy to provide referrals whenever possible.
